The Right HGH Dose: Why IGF-1, Not IU, Is the Number That Matters
Most people ask how many units of growth hormone they should take, and that question is backwards, because units are just the input and IGF-1 is the output, and the output is the only thing your body actually responds to.
Here is the full chain before we zoom into any piece of it.
You inject growth hormone. It travels to the liver, and the liver responds by producing something called IGF-1, which is insulin-like growth factor 1, the hormone that actually carries out most of what people credit growth hormone with doing. IGF-1 then circulates and tells tissue to build, repair, and grow.
So growth hormone is the signal, and IGF-1 is the answer that comes back. When you dose HGH, you are really just turning a dial on how loudly your liver shouts IGF-1 into your bloodstream.
That distinction matters because two people taking the same dose can land in very different places. Liver function, body fat, insulin sensitivity, age, and how much natural growth hormone you still produce all shift how much IGF-1 a given number of IU generates.
Now to the number itself.
If you're staying below 4 IU per day, your IGF-1 levels aren't really gonna go much greater than 400. And when we talk about the risk that's associated with your IGF-1 levels, it's really above 400 where you start to see problems in terms of risk reward, right? For the average guy who's just looking to optimize his health, feel good, perform better, a dose of around two to four IU per day is going to get your IGF-1 levels somewhere between two and 400. Those are ranges from what I have seen in practice, and I cannot point you to a study that draws a hard line at 400 or maps IU to IGF-1 that cleanly.
Why does anyone care about the ceiling at all? Because IGF-1 does not distinguish between tissue you want to grow and tissue you do not. It is a growth signal, full stop. Muscle reads it, connective tissue reads it, and cells that are already dividing abnormally read it too.
Think of it like fertilizer on a lawn. Spread the right amount and the grass thickens. Dump the bag out in one spot and you feed everything in that soil, including whatever you were not trying to grow.
That is the whole logic of staying in a moderate range instead of chasing the highest number you can tolerate.
Your goal should be between three and 400, keeping in mind that your testosterone is already optimized too. And those two things together, working synergistically, is really what's gonna give you the biggest bang for your buck for sleep quality, performance, strength gain, muscle gain, fat loss, tissue repair. That pairing is what I have seen produce the best results in clients, and no study has shown that combination beats either hormone alone in a controlled setting.
The reason testosterone keeps coming up in a conversation about HGH is that growth hormone without adequate androgen support tends to underdeliver. Testosterone drives the anabolic machinery, and IGF-1 supports the repair side. Running one high and the other low wastes the one you are running high.
Practically, start low, run bloodwork at six to eight weeks, and adjust from what your IGF-1 actually reads rather than from what you assumed the dose would do.
So with all that said, is there a risk associated to optimizing your growth hormone IGF-1 levels? Yes, of course, there's a risk to everything, right? But if your goal is to optimize, if you just stay within that two to four IU per day for men, one to two IU for women, you're probably not gonna have a big problem. From what I have seen, that lower range works for women without pushing IGF-1 anywhere concerning, and the data is not there to confirm a specific safe threshold for either sex.
