The Sauna Isn't Doing What You Think

Your liver processes roughly 1.4 liters of blood per minute, filtering out waste products, metabolizing hormones, and neutralizing compounds your body has flagged as toxic. Your kidneys filter about 180 liters of fluid every single day. When you sweat in a sauna, you produce maybe one liter of fluid per hour, and that fluid is over 99% water. The detoxification claim falls apart the moment you look at what each system is actually doing, and it turns out the same logic problem applies to most of what people believe the sauna is doing to their hormones.

So let's start with testosterone, because that belief is everywhere.

The idea usually goes something like this: heat stress is a stressor, stressors trigger adaptation, testosterone is an anabolic hormone tied to adaptation, therefore heat should raise testosterone. The logic sounds reasonable. The data does not support it.

In 1986, researchers put 17 volunteers through a dry sauna set to 80 degrees Celsius, which is about 176 degrees Fahrenheit, for one hour twice a day for seven days. At the end of that week, there were no statistically significant changes in testosterone, FSH, or LH. FSH and LH are the upstream signals the brain sends to the testes to produce testosterone, so the researchers were checking the whole chain, not just the end product. The chain did not move.

But something else in that study moved dramatically.

Growth hormone went up 16 times above baseline in the male participants. That number deserves some context. Exercise can raise growth hormone. Cold exposure can raise growth hormone. The response from this sauna protocol was larger than what most of those stressors produce, which means the sauna is not useless for hormones, it is just useful for a different one than most people assume.

Growth hormone does a lot of what people credit testosterone for: it supports muscle repair, body composition, and recovery. So the sauna is still a tool for those goals, just through a different mechanism than the one people usually picture.

Now here is where the practical details matter. The protocol that produced that 16-fold response was not a casual fifteen-minute warm-up. It was four 30-minute sessions in a single day, separated by 5-minute cool-downs, on an empty stomach, in a dry sauna at 176 to 212 degrees Fahrenheit. That is a specific, demanding protocol.

There is also an adaptation problem. Your body is very good at adjusting to repeated heat stress, which means the hormonal response shrinks the more frequently you do this. The growth hormone spike is essentially your body's emergency response to an unusual thermal challenge, and once it stops being unusual, the response dials down. That is why this protocol produces its best results when used once a week or less. Doing it every day does not give you more benefit, it trains your body to treat the heat as normal, and the signal disappears.

That adaptation principle is important to understand because it is precisely why the cardiovascular data tells a completely different story about frequency.

A 20-year study following 2,315 Finnish men aged 42 to 60 found that men who used the sauna four to seven times per week had a 63% lower risk of sudden cardiac death compared to men who went only once per week. They also had a 40% lower risk of all-cause mortality. Sessions longer than 19 minutes were associated with significantly better outcomes than shorter sessions.

The mechanism here is not hormonal. The cardiovascular benefit comes from something called repeated passive heat exposure, which is a way of training the cardiovascular system through sustained increases in heart rate and cardiac output without physical movement. Each sauna session functions like a mild cardiovascular workout. Your heart rate rises, your blood vessels dilate, your plasma volume adapts over time. Done frequently, these adaptations accumulate. The benefit is cumulative and it requires consistent repetition, which is the opposite of what produces the growth hormone effect.

The same cohort of men from that cardiovascular study showed a 66% lower risk of dementia and a 65% lower risk of Alzheimer's disease in the group using the sauna four to seven times per week. The proposed mechanism involves improved cerebrovascular circulation, reductions in inflammatory markers, and the cardiovascular adaptations mentioned above, though it is worth noting that large observational studies like this one show association, not proven causation. The men using the sauna more frequently may have also lived differently in other ways. The signal is strong and consistent, but the mechanism is still being worked out.

What this means practically is that you are choosing between two different tools that happen to share the same equipment.

For cardiovascular and cognitive benefits, frequency is what matters. Four or more sessions per week, at least 19 minutes per session, used consistently over years. The body needs repetition to accumulate the circulatory adaptations.

For growth hormone, infrequency is what matters. Once a week or less, longer sessions, on an empty stomach, with the cool-down intervals that appear to be part of what drives the response. Use it too often and the adaptation effect erodes the signal.

Most people using the sauna casually are getting neither benefit fully because they are doing it often enough to blunt the growth hormone response but not often enough to accumulate the cardiovascular adaptations. Picking a goal and matching the protocol to it is the part people skip.

The sauna is a tool that does real things to the body. It just does them through mechanisms that have nothing to do with the reasons most people cite. And the correct protocol for one goal is almost exactly the wrong protocol for the other, which means the first question is not how often, but what for.
