Your Optimal Testosterone Dose Is the Highest One That Doesn't Need an AI
Okay, this is a good question and I actually brought my notes into the gym.
And this is specifically for men, is how do I find my optimal testosterone dose? I can't tell you how many times we get guys who come back and they say, hey, my testosterone level is this, my total T is this, my SHBG is this, you know, my estrogen is this, what should I do? You should be doing as much testosterone as you can without needing an AI. Okay, that's the key.
Before that makes sense you need the chain, because testosterone does not just sit in your blood doing one job. Some of it binds to something called SHBG, which is sex hormone binding globulin, a protein that holds testosterone so your tissues can't use it, and some of it stays free instead. A portion of the free testosterone gets converted by an enzyme called aromatase into estradiol, which is your primary estrogen, and that estradiol is doing real work rather than sitting there as a byproduct.
Estradiol in men regulates bone density, supports joint health, helps control lipids, and drives libido and erectile function. So when you raise your dose, estradiol rises with it, and that is the system working correctly.
The problem starts when someone sees a number on a lab printout and decides to fight it.
An aromatase inhibitor, or AI, blocks that conversion enzyme, and it does the job with no precision at all. You take it because your estradiol reads high, and it drives estradiol down, and it does not know where to stop.
There are TRT clinics who are prescribing replacement doses, putting their patients on AIs and it's literally destroying their long-term health.
Okay, if you are crashing your E2 and treating this symptom is doing nothing but creating negative long-term health effects to not only your bone health, your heart health, but your performance, the results that you're going to see in the gym, etc.
Think about it like the oil pressure in an engine. A high reading might mean something is wrong upstream, but draining the oil doesn't fix the problem, it just wrecks the engine quietly while the gauge looks better.
So the actual method is a ceiling test rather than a target chase.
You find the highest testosterone dose where you still feel good, still have libido, still sleep, still recover, and your estradiol rises but doesn't produce symptoms. That's your number. The moment you need a second drug to manage the side effects of the first one, the dose is the problem.
Which means you titrate down instead of medicating sideways. Bring the weekly total down. Or split the same weekly amount into more frequent injections, because smaller doses produce flatter peaks, and peaks are what drive aromatase hardest.
Most of the guys who think they have an estrogen problem have an injection frequency problem, and going from one shot a week to two or three often clears symptoms with zero change in total dose. That's what I see in practice, not something I'm pulling from a trial.
Two things worth checking before you touch the dose at all. Body fat, because aromatase is concentrated in fat tissue, so more fat means more conversion at the same milligram amount. And alcohol, sleep and thyroid, all of which change how you feel in ways men reliably blame on estrogen.
How you actually feel carries more weight than the number on the page. A man at 60 pg/mL who feels great is fine. A man at 35 with dead libido, aching joints and flat mood is probably crashed, and his lab work looks perfectly respectable.
The dose you can carry without a rescue drug is the dose your body is actually built to hold. Everything above that is borrowed, and the AI is the interest payment.
