Keto Weight Stall
Most people starting a ketogenic diet lose 10 to 15 pounds in the first two weeks, and then the scale barely moves. That second phase feels like failure. It is actually the opposite.
To understand why, you need to understand two completely different systems your body uses to store and release energy, because the first phase and the second phase are governed by entirely separate biology.
Your body has a short-term energy storage system and a long-term one. The short-term system runs on something called glycogen, which is glucose that your liver and muscles have bundled together into dense chains and stored for quick access. Think of glycogen as the cash in your wallet. It is immediately spendable but there is only so much room to carry it. The long-term system is body fat. That is the money in the bank. Slower to access, but the storage capacity is enormous.
When you cut carbohydrates, your body burns through the wallet first.
You store roughly 500 grams of glycogen at any given time, split between your liver and your skeletal muscle. That number on its own does not sound like much weight. But glycogen does not travel alone. Every gram of glycogen holds somewhere between 3 and 4 grams of water bound to it, which was confirmed in research going back to Olsson and Saltin in 1970 and replicated again in more recent work by Schytz and colleagues in 2023.
Do the math. 500 grams of glycogen, times 3 to 4 grams of water, gives you somewhere between 1,500 and 2,000 grams of water released the moment glycogen is depleted. That alone is roughly 4 to 5 pounds of water weight, and it comes off fast, often in the first several days.
Then insulin drops.
When carbohydrate intake falls, insulin levels come down significantly, and lower insulin tells your kidneys to stop holding onto as much sodium, which causes your kidneys to excrete more water along with it. On top of that, in the early days of ketosis your body produces ketone bodies faster than it can use them and excretes the excess through urine, which pulls additional water with it. These two mechanisms layer on top of the glycogen water loss and together push that initial total to the 10 to 15 pound range that most people experience.
This is what Kreitzman and colleagues called the illusion of easy weight loss in a 1992 paper in the American Journal of Clinical Nutrition. They were not saying the weight loss is fake. They were saying it is a different thing than what most people think they are measuring. Those 10 to 15 pounds are real. They left your body. But they were not stored energy. They were stored water, and water comes back the moment glycogen comes back, which is exactly why people who go back to eating carbs after a week of keto see the scale reverse almost overnight.
The slowdown that happens after that initial phase is not a plateau. It is a transition.
Your body has now moved from burning through its short-term stores to drawing on the long-term system, which means actual adipose tissue, actual stored fat. And fat is a fundamentally different energy substrate than glycogen. A pound of fat tissue contains approximately 3,500 calories of stored energy, a number that comes from Hall's 2008 analysis in the International Journal of Obesity. That is a lot of energy packed into a single pound, which is exactly the point of fat as a storage molecule, but it means that burning through it takes sustained time and a sustained caloric deficit.
At a modest deficit of 500 calories per day, which is roughly what most people are running on a well-formulated ketogenic diet without deliberately restricting calories, you are looking at about one pound of fat lost per week. At a larger deficit, maybe twice that. Neither of those numbers will impress you on a bathroom scale when the previous two weeks were showing several pounds dropped every few days.
But here is what the scale cannot show you. Fat loss and weight loss are not the same measurement.
When your body burns stored fat, the fat cells themselves do not simply disappear. The fat exits the cell, gets broken down through a process called beta-oxidation, and the carbon leaves your body through your breath as carbon dioxide and the hydrogen leaves largely as water vapor and urine. The cell shrinks. The tissue compresses. Your body composition is changing in a way that reshapes your measurements even before the scale moves significantly.
This is why a tape measure tells you more than the scale does during this phase. Your waist, your hips, your thighs are likely changing even in weeks where your weight holds flat, because fat tissue is less dense than muscle and takes up more volume per pound, so losing even a small amount of it produces visible and measurable changes in circumference before the numbers accumulate enough to register as significant weight change.
The people who interpret the slowdown as failure are measuring the wrong thing at the wrong time, and they often quit right at the moment the process actually started working.
The first two weeks of keto are your body clearing the structural water out of your system. The following weeks are your body burning its actual stored fuel. These are not two phases of the same process. They are two entirely different biological events that happen to occur in sequence, and the only mistake you can make is treating the slower second event as evidence that the first event was a fluke.
