Does Tongkat Ali Actually Boost Testosterone?

Tongkat ali is one of those supplements that has been around long enough to accumulate real research behind it, and that research tells a more specific story than the marketing does.

To understand what it actually does, you need to understand the system it is working inside.

Your testosterone levels are not a fixed number. They are the output of a constant balancing act between production signals coming down from your brain, conversion happening in your tissues, and suppression coming from stress hormones. When any part of that system is out of balance, your testosterone drops below where it should be. Tongkat ali, specifically a compound inside it called eurycomanone, targets two points in that system at once.

The first is something called aromatase, which is the enzyme responsible for converting testosterone into estrogen. This conversion happens naturally and some of it is necessary, but when aromatase activity runs too high, you are essentially losing testosterone faster than your body can make it. Eurycomanone inhibits aromatase activity directly, which was demonstrated in lab research showing the compound reduces testosterone-to-estrogen conversion at the cellular level.

The second point is cortisol. Cortisol is your primary stress hormone, and one of its effects is that it directly suppresses testosterone production at the level of the testes. When your cortisol is chronically elevated because of poor sleep or ongoing psychological stress or overtraining, your body is essentially running a signal that says this is not a safe time to invest resources in testosterone production. Tongkat ali appears to reduce cortisol, and when cortisol comes down, that suppressive signal weakens and testosterone has room to recover.

Those two mechanisms together, less aromatase conversion and less cortisol suppression, explain most of what the research actually shows.

A 2013 study put 63 moderately stressed adults on 200 milligrams of tongkat ali daily for four weeks and measured what happened. Cortisol dropped 16 percent and testosterone went up 37 percent. That is a meaningful number but it also tells you something about the mechanism. The cortisol drop came first and likely drove most of the testosterone increase. This was not the compound directly stimulating testosterone production so much as it was removing two things that were pulling testosterone down.

A review and meta-analysis of five clinical trials published in 2022 did find a statistically significant improvement in total testosterone across those studies, with an effect size that indicates a real signal in the data. But when you look at where that signal came from, it was concentrated in men who were hypogonadal, which means men whose testosterone was already clinically below the normal range.

The clearest illustration of that is a 2012 open-label study that gave 200 milligrams daily to 76 men with confirmed low testosterone. After one month, over 90 percent of them had testosterone levels back in the normal range. That is a large and consistent effect. It also had no placebo control group, which means you cannot rule out that some of those men would have normalized on their own over that period, so the number needs to be held with that in mind.

Here is where the story gets more precise. When researchers ran a properly controlled trial, 109 men aged 30 to 55, randomized, placebo-controlled, 300 milligrams daily for 12 weeks, they did not find a meaningful increase in total testosterone compared to placebo. This was the largest and most rigorous study in the set and it found essentially no effect.

The men in that study were not selected for low testosterone. They were aging males, some of whom presumably had normal levels and some of whom were likely lower, but it was not a hypogonadal sample the way the earlier studies were.

That gap between results is actually the finding. Tongkat ali does not appear to push testosterone above its natural ceiling in men who are already functioning normally. What it appears to do is correct for the specific things that are dragging testosterone below where it should be, which is aromatase running too high and cortisol running too high. If neither of those is a meaningful problem for you, the mechanism does not have much to work with.

This is the distinction between deficiency correction and enhancement, and most supplement marketing blurs it completely. Correcting a deficiency can produce changes that look dramatic in the data because you are moving someone from below normal to normal. That is real and meaningful if you are the person below normal. But it is not the same as a compound that raises testosterone in healthy men with no underlying suppression.

The one practical note on side effects is that tongkat ali does have mild stimulant properties, which means some people experience disrupted sleep or a wired feeling, particularly at higher doses or when taking it later in the day. If that happens it is a signal to either reduce the dose or move it earlier in your schedule.

The actual takeaway is narrower than the marketing suggests but more meaningful for the right person. If your testosterone is below the normal range, especially if your stress is elevated or your lifestyle has been running cortisol high, the evidence suggests tongkat ali may genuinely help bring you back to baseline. If you are already in a healthy range with well-managed stress, the most rigorous study available found nothing meaningful. Same supplement, two completely different outcomes, and the difference is entirely about where you are starting from.

The supplement is not the variable. Your baseline is.
