Does Tongkat Ali Actually Boost Testosterone?

Tongkat ali sits in an interesting position in the supplement world, because the research on it is real, it just does not say what most people selling it want you to believe it says.

To understand what the compound actually does, you need to understand the system it operates in, because tongkat ali does not create testosterone out of nowhere. It works by removing two things that are actively suppressing the testosterone you already have the capacity to make.

Your body produces testosterone through a chain of signals that starts in the brain, runs through a gland at the base of your skull called the pituitary, and lands in the testes where testosterone is actually synthesized. Once testosterone is made, two things can bring it back down. The first is something called aromatase, which is an enzyme that converts testosterone directly into estrogen. The second is cortisol, which is your primary stress hormone and which directly suppresses the signaling that triggers testosterone production in the first place. So even if your brain is sending the right signals, high cortisol can interrupt the chain before it ever reaches the finish line.

Tongkat ali works on both of these suppressors, and that is where the active compound comes in.

The compound responsible for most of the effects is something called eurycomanone, which is a bitter-tasting molecule found in the root of the Eurycoma longifolia plant. In lab studies, eurycomanone has been shown to inhibit aromatase activity, meaning it slows the conversion of testosterone into estrogen, so more of the testosterone you produce stays as testosterone. It also appears to lower cortisol, which removes the brake that stress hormones put on your testosterone production. When you take both of those effects together, what you are doing is clearing roadblocks, not building new roads.

That distinction matters when you look at what the studies actually found.

A 2022 systematic review and meta-analysis looked at five randomized controlled trials and found that tongkat ali did produce a statistically significant increase in total testosterone, with a standardized mean difference of 1.352 across the studies. That is a meaningful effect size on paper. But when you look at who was in those studies and what was happening to their testosterone to begin with, the picture becomes more specific.

The strongest result in the literature is an open-label study where 76 men with clinically low testosterone took 200 milligrams of a standardized tongkat ali extract daily for one month. By the end of the month, 90.8% of those men had testosterone levels that had returned to a normal range. That is a striking number, but there was no placebo group in this study, so you cannot fully isolate how much of the change was the compound versus other variables, and the authors acknowledged that limitation.

The cortisol angle gets its own dedicated study. In 63 moderately stressed adults who took 200 milligrams daily for four weeks, cortisol dropped by 16% and testosterone increased by 37%. That is a meaningful shift, and it strongly suggests that a significant portion of tongkat ali's testosterone effect is being mediated through cortisol reduction rather than direct hormonal stimulation. If your stress hormones are suppressing your testosterone, and you lower the stress hormones, testosterone comes up. That is not tongkat ali boosting testosterone so much as tongkat ali removing the thing that was pulling testosterone down.

Now here is the part that changes the practical value of the supplement entirely.

The largest placebo-controlled trial in the literature took 109 men between the ages of 30 and 55 and gave them 300 milligrams daily for 12 weeks. This was a properly controlled study with a placebo group. The result was no significant change in total testosterone compared to placebo. These were men whose testosterone was in a generally healthy range to begin with, and the supplement did not move it meaningfully.

When you put all of this together, a pattern emerges. Tongkat ali appears to work by relieving suppression. If your testosterone is low because of elevated cortisol, because of chronic stress, or because of some degree of aromatase overactivity, removing those suppressors gives your system room to recover and your levels can come back up toward where they should be. But if your testosterone is already operating without significant suppression, there is no roadblock to remove, and the compound has very little to push against.

This is what deficiency correction looks like versus actual enhancement. Think of it like a car engine that is running rough because the air filter is clogged. Cleaning the filter lets the engine run the way it is supposed to run, and you might feel a significant difference. But if the filter was clean to begin with, cleaning it again does not make the car faster.

Practically speaking, if your testosterone is clinically low and your stress levels are high, there is reasonable evidence that tongkat ali at 200 to 300 milligrams daily could help bring your levels back toward a normal range. The effect appears within weeks based on the existing studies. If your testosterone is already in a healthy range and your stress is managed, the best-controlled study does not support expecting a meaningful increase.

One thing worth noting on the side effect side: tongkat ali has stimulant properties, and some people report difficulty sleeping or a restless feeling at higher doses or when they take it late in the day. This is not a sign the supplement is working particularly well. It is more likely a sign the dose is too high or the timing is off.

The deeper takeaway from all of this is that most of what gets marketed as testosterone boosting is really testosterone restoring, and those are not the same thing. Your body has a natural operating range for testosterone, and most compounds, including tongkat ali, can only help you get closer to the top of your own range if something is pulling you below it. No supplement changes where your ceiling is. It can only help you stop living at the floor.
