Does AOD-9604 Actually Burn Fat? (What the Phase 2 Trial Showed)

Does AOD-9604 Actually Burn Fat?

AOD-9604 failed its own phase two trial, and the placebo group lost more fat than the group taking the peptide. That is the entire story, and it is the reason I don't use it with anyone.

But the reason I don't recommend AOD is for two reasons. The first is what the trial data actually showed, and the second is what people are doing with it anyway.

Start with where the hype came from, because it did not come from nowhere. AOD-9604 is a fragment of human growth hormone, specifically the tail end of the molecule, the part that researchers believed carried the fat-burning signal without the blood sugar effects or the tissue growth that comes with full growth hormone.

That idea is reasonable on paper. Growth hormone does drive lipolysis, which is the breakdown of stored fat into free fatty acids the body can burn, and if you could isolate the stretch of the molecule responsible for that one job, you would have a fat loss compound without the rest of the baggage.

And in the early work, it looked like it was going somewhere. They saw a meaningful fat loss benefit from the people who had took it versus the placebo.

This was a small controlled study where they were able to say, hey, yeah, like this looks like it works.

That is exactly how drug development is supposed to go. A small trial generates a signal, the signal looks clean enough to justify more money, and then you scale it up. Let's move it to phase two where we're gonna expand the control group and see if this works on a larger population.

The reason you scale up is because small trials lie. With thirty or forty people per arm, random variation in who happened to be dieting harder that month can look identical to a real drug effect, so the only way to know if the signal is the compound or the noise is to run it on enough people that the noise averages out.

When they tested AOD-9604 on the larger population during the phase two trial, the placebo outperformed the control.

Meaning the people who were taking AOD or thought they were taking AOD but weren't getting anything at all lost more fat than the people who were. And this was a study of like, I believe over 700 people.

That number matters more than most people realize. At seven hundred plus participants, you are no longer looking at a trial that failed because it was underpowered or because the sample was weird. You are looking at a trial large enough to detect a small effect, and it detected nothing, and the direction it did move was away from the drug.

The honest read on that is simple. The lipolytic signal that showed up in the small study was noise, and the placebo arm did what placebo arms in weight loss trials always do, which is lose weight because people in a monitored study eat better and move more when someone is weighing them every two weeks.

So if you're taking AOD-9604, there's no controlled clinical data to demonstrate that this even fucking works.

What I see in practice lines up with that. People running AOD for twelve weeks report fat loss, and when I ask what else changed, there is always a calorie deficit, a new training block, or a GLP-1 sitting underneath it. The peptide gets the credit for the thing the deficit did.

The practical move is not complicated. If you are spending two hundred dollars a month on AOD-9604, that money buys a better outcome almost anywhere else, whether that is a coach who audits your intake, a continuous glucose monitor so you can see what your food is doing, or an actual GLP-1 agonist that has phase three data behind it.

And if you are already on it and losing fat, keep everything else exactly the same and drop the AOD for six weeks. If the rate of loss does not change, you have your answer, and it is the same answer the seven hundred person trial gave.
